Whistleblower Submission

  1. Purpose

    Independent Living Assessment (iLA) encourages individuals to come forward with any concerns regarding misconduct or wrongdoing within the organisation. This form is intended to assist you in submitting a whistleblower disclosure.

    Please be aware that you may be asked to provide further assistance during any subsequent investigation, if necessary. For more details, please consult iLA’s Whistleblower Policy, which is available on our website.

  2. Are you an Eligible Discloser?

    Under the Corporations Act an eligible whistleblower is anyone with a current or former connection to iLA — such as employees, officers, suppliers, associates, or their relatives or dependants — who reports suspected misconduct.

    We’ll consider your information to determine whether you are eligible for protection.

    If you are a client who has received a service from iLA, your disclosure will be treated as a confidential complaint. The Corporations Act 2001 does not include a customer/client relationship on its eligibility list for whistleblower protections, unless they meet the above relationship criteria.

  3. Reporters Contact Information

    You may choose to make your disclosure anonymously, and your identity will be protected in accordance with the Corporations Act 2001.

    On submission, this report form is securely encrypted and is only accessible by the CEO and Board Chair.

    This form can also be printed and posted anonymously to us via the mailing address detailed in our Whistleblower Policy.

  4. What happens after you make an eligible disclosure with iLA?

    If you have given us your contact details, we will contact you to acknowledge receipt, and discuss the matter within 3 days, any further information we may need, and any steps we may take to investigate it.

    iLA requires that a protected disclosure is dealt with within 60 days from the date it is received. You can generally expect that the matter will be completed in this period. We will advise you if the investigation is likely to take longer.

You can remain anonymous, but this may limit any investigation of the disclosure, if we are unable to contact you to obtain more information as we progress our investigation. We will also not be able to notify you of the outcome of the investigation. Alternatively, you could provide us with your name and contact details and ask that we don’t reveal them to others. This will help us investigate your concerns, and also protect your identity.
