THE ROLE

We are currently seeking a Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker with aged care and/or leadership experience to help support our Aged Care Assessment Services, in the position of Aged Care Assessment – Team Lead.

Key responsibilities of this role include:

  • Lead a team of Assessors across metropolitan, regional and remote Western Australia by coordinating, overseeing and planning delivery of aged care assessment services.
  • Building team capability and ensuring assessments are completed on time and to a high standard.
  • Ensure high client satisfaction by monitoring assessment quality, addressing issues promptly, and driving service improvements.
  • Ensure work practices / systems that are conducive to productivity, safety, continuous improvement, and employee engagement.    
  • Coordinate Assessor resourcing and service delivery with relevant stakeholders to meet operational needs and KPIs.
  • Supervise team members by mentoring and coaching, supporting development and training, and allocating duties.
  • Ensure the team delivers culturally safe, inclusive and (where appropriate) trauma-informed assessments and communications.
  • Promote healthy ageing and well-being using a ‘wellness and reablement approach’. 
  • Conduct Aged Care Assessments, Triage and Delegation services as required.

ABOUT YOU

Our ideal candidate will possess aged care experience, strong leadership capabilities, a collaborative team centred approach, excel in meeting operational requirements and maintain a commitment to high-quality outcomes.

Essential requirements

  • A tertiary qualification in Nursing, Social Work, Occupational Therapy, Physiotherapy or a related health field and current unrestricted registration with AHPRA or eligible to be part of a relevant professional association
  • Demonstrated capability to manage day-to-day workflow, capacity planning and allocation to meet service targets
  • Experience using data, audit findings or client feedback to drive quality improvement and continuous improvement initiatives
  • A passion for promoting independence and well-being among older adults
  • Excellent interpersonal and communication skills, both written and verbal
  • Ability to work flexibly and complete aged care needs assessments, triage and/or delegation duties as required
  • Current driver’s licence and willingness to travel across WA as required
  • Willingness to comply with any vaccination or health requirements applicable to the role
